Do CDL drivers have to attach their medical certificate to their CDL and provide to the state licensing departments?

Yes. As of January 30, 2014, all non-exempt interstate and intrastate CDL holders are required to complete a Medical Certification Information Sheet. Failure to do so may mean loss of commercial driving privileges.
Drivers must take the Medical Certification Information sheet and a copy of their medical certificate to the State Licensing Department, where the information will be added to their CDL. The process must be repeated each time medicals are renewed.
